NSIP


Prone HRCT in a patient with ILD shows a reticular pattern in the posterior and subpleural lungs. The reticular opacities outline focal rounded lucencies (arrows) which may represent small honeycomb cysts or small airways dilated because of surrounding fibrosis (so-called traction bronchiolectasis). The presence of honeycombing would favor the diagnosis of usual interstitial pneumonia (UIP) or idiopathic pulmonary fibrosis (IPF). The presence of traction bronchiolectasis without honeycombing is nonspecific and is compatible with UIP and IPF or fibrotic nonspecific interstitial pneumonia (NSIP). This case represents fibrotic NSIP.
