Parenchymal chronic inflammation


A few scattered areas of more intense parenchymal chronic inflammation are present in the biopsy, around terminal airways. This appearance is more in keeping with classical "LIP." Lymphoid germinal centers (GC) are typically present and constitute so-called "follicular bronchiolitis." The terminal airway is highly remodeled here showing many irregular luminal profiles ("peribronchiolar metaplasia;" PBM). Scattered multinucleated giant cells (arrow) are an expected finding in LIP; sometimes small non-necrotizing granulomas can occur without identifiable infection. When this degree of lymphoid hyperplasia is present in the lung, ancillary studies (immunohistochemistry and molecular techniques) are warranted, to exclude lymphoma.
