This low magnification image shows the diffuse accumulation of macrophages in alveolar ducts and alveoli that defines this idiopathic interstitial pneumonia. The alveolar ducts (ad) and alveoli are filled with plump macrophages (m) typically showing smoking-related, light brown pigmentation. A few small lymphoid aggregates (L) and a terminal bronchiole (br) are present.
