Prone HRCT through upper lobes typical of a smoker with Langerhans cell histiocytosis

Prone HRCT through the upper lobes shows the cysts (arrows) to be larger, irregular in shape, and occupying a greater cross-sectional area of lung. This upper lobe predominance would also be distinctly unusual in UIP/IPF. This appearance of irregular lung cysts with an upper lobe predominance is typical of Langerhans cell histiocytosis in a cigarette smoker.
