Prone HRCT near lung bases of a smoker with Langerhans cell histiocytosis


Prone HRCT near the lung bases in a 36-year-old smoker with dyspnea shows a combination of reticular opacities and small lung cysts with easily definable walls (arrows). Although this appearance resembles honeycombing in UIP and IPF, the cystic spaces predominate in the central lung regions and the posterior and subpleural lung is relatively spared. This would be very atypical for honeycombing in UIP/IPF.
