55-Year-Old With Shortness of Breath and Lower-Leg Swelling

CLINICAL INFORMATION

Past Medical History

Hypothyroidism, pneumonia, gastroesophageal reflux, chronic bladder problems, and an episode of jaundice.
Allergies: Penicillin and sulfa.
Social History: One pack of cigarettes daily; occasional alcohol.
Family History: No history of lung disease.
Vital Signs: Blood pressure: 130/88 mm Hg; pulse: 80 bpm.

Physical Examination

General: Obese female in no acute distress; appeared somewhat anxious.
HEENT: TMs clear; nares without polyps or rhinitis; pharynx clear.
Neck: Without adenopathy.
Chest: Distant breath sounds, no crackles or wheezes.
CV: Regular rate, no murmur or gallop.
Abdomen: Abdomen is obese, nontender; no masses or organomegaly; normal.
Extremeties: 1+ edema in both ankles noted. It is symmetric.
Neurologic: Grossly intact.
Skin: No suspicious lesions seen.
