CASE STUDY:
64-Year-Old Hispanic Female With Shortness of Breath, Weight Loss, and Diarrhea

CLINICAL INFORMATION

HISTORY OF PRESENT ILLNESS

A 64-year-old, previously healthy Hispanic female presented 5 years prior with shortness of breath, weight loss, and diarrhea. The initial chest x-ray showed nodular opacities and no mediastinal lymphadenopathy. Subsequently, a noncontrast chest CT scan revealed 2–3 mm nodules and reticular infiltrates.

ADDITIONAL DATA

Room air pulse oximetry was 97%. An echocardiogram revealed normal left ventricular systolic function with no evidence of pulmonary hypertension.

PULMONARY FUNCTION TESTS

Table 1
FEV1 FVC FEV1/FVC
2.16 L (86%) 2.33 L (73%) 93%

Spirometry was consistent with a mild restrictive deficit (see Table 1). Cardio-pulmonary exercise test (CPET) showed normal anaerobic threshold (AT) of 0.663 L/min and VO2 max of 1.409 L/min.
